Nomination Entry Form

Nominate a deserving mother or mother figure to have a chance to win the following:
  • Photo Shoot by April Scott Photo
  • Make Over by the talented Abigail Trufant of Divinity Salon, the day of the photo shoot,       to include:
    • Haircut 
    • Color
    • Style, AND 
    • Make Up.

 After the photo shoot they will receive:
  • An Accordion Album of your session and a few digital files. 
  • As well as the nomination letter printed in a picture frame. 

Let us treat your loved one like a QUEEN for the day.

WINNERS to be announced on Mother's Day through Facebook and winners will be contacted directly.
